Health Promotion and Chronic Disease Capacity Building Grants

Overview

The West Virginia Bureau for Public Health Division of Health Promotion and Chronic Disease (HPCD) takes a comprehensive approach to chronic disease management and prevention, working with communities, health systems, and decision-makers to make it easier for people to be health in the places they live, learn, eat, work, and play. West Virginians will need to work together to make the greatest impact on improving health outcomes.

It is widely recognized that a community driven approach to health promotion that engages key stakeholders is an effective way to prevent or reduce the impact of chronic disease. HPCD provides grant funding to several organizations across West Virginia –Active Southern West Virginia, Center for Active West Virginia, the WVU Office of Health Services Research,  Mountains of Hope Cancer Coalition, and Marshall University Minority Health Institute. In turn, these organizations administer grants to schools, worksites, university departments, community groups, and clinical entities throughout West Virginia. Drawing from multiple sources of funding, the HPCD Capacity Building Grant Program addresses obesity and the prevention and management of chronic disease. The grant program encourages grant recipients to implement policy, systems and environmental (PSE) changes to address physical activity, fruit and vegetable consumption, reduced sugar sweetened beverages, increased water intake, and tobacco control.
