Chronic Disease Self-Management Program

Participants make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. 

Program Goals

  • learn problem-solving

  • decision-making

  • techniques for managing common problems 

  • goal-setting for the upcoming week and develop an action plan for meeting that goal

  • develop support system within group

Participants gain skills such as:

  • addressing physical and psychological impacts of chronic disease 

  • exercise, developing healthy eating habits, and using medications appropriately

  • communicating effectively with family, friends, and health professionals
